Monthly Membership
Followup visits are for established patients who completed an initial exam & visit with provider.
Service Description
Monthly membership includes regular Followup Visits on a 4 week (28 day) recurring schedule. If more frequent or additional visits are required, extra visits will be invoiced as needed. Followup visits are conducted by a Board Certified Nurse Practitioner (NP). NP's have additional advanced training in the diagnosis and treatment of painful conditions. Please note: Provider options are limited for patients who have not completed an Initial Evaluation. So that we can best assist you, new patients must schedule an Initial Visit before scheduling a Followup Visit. Payment is for the time & expertise of your healthcare providers and does not guarantee medication. OnCall Pain does not participate with any insurance carriers. All services are non-covered services and patients are solely responsible for payment.
Cancellation Policy
Due to the time and expense of the new patient intake process & reserving time with your provider, Initial Visit fees are non-refundable. Followup Visits may be cancelled anytime prior to your scheduled visit and a refund may be issued for the cost of the visit minus credit card fees. If you can't attend your scheduled appointment, please cancel or reschedule before your appointment. Missed appointments may be charged a No Show Fee equal to your visit charge amount. Credit card processing fees are non-refundable. A 3% fee will be withheld from all refunds to cover non-refundable credit card expenses.
